The heart in this image was created using the shape tool in Photoshop with a red fill the shape was then rasterized. Various texturing filters were used as well as the some use of the burn and dodge tools. The arm was from a separate photo and had similar processing. The background was created by rendering clouds and applying various distortion filters like twirl, as well as motion blurs.
