A number of investigations looked at the explosion and spill’s cause. Importantly, the September 2011 report by the U.S. government highlighted to poor quality cement work done on the well. The same report’s conclusions found BP to be primarily at fault and also faulted Transocean, the operate of Deepwater Horizon operator, and Halliburton, the contractor. Some time earlier in 2011, a White House commission also put the blame on BP and the company’s partners for executing a series of actions to lower costs and maintaining insufficient well safety systems. The commission’s report also determined that the oil spill was an unisolated event brought about by “rogue industry or government officials”. These were caused by “systemic” root problems and, “absent significant reform in both industry practices and government policies, might well recur.” Triggering more than 130 private lawsuits, the disaster led to civil and criminal federal prosecutions. By November 2012, BP agreed to settle the federal case, in which they pled guilty to 11 counts of manslaughter linked to the explosion and fire. BP agreed to payout $4.525 billion in fines and other payments, a number which broke records at the time. BP faced more possible gigantic payments to thousands of businesses, fishermen and other people harmed by the oil spill. The EPA stated in November 2012 that BP would be temporarily prevented from developing new US government contracts, due to the “lack of business integrity” by the company as the oil spill occurred.
